Care Homes with Nursing
Our Community Based Complex Care Services are developed in partnership with a Local Authority and or Clinical Commissioning Group to meet an identified local demand and with specific individuals in mind, who require a high level of support to live within a community setting. These services are for individuals from that specific area to enable them to be repatriated closer to home.
We provide a specialist community based provision with individual bespoke care packages which are person-centred, and outcome-focused which enables people with continued behaviours of concern and high support needs to be cared for in the least restrictive setting possible, but within a service which is bespoke and structured enough to meet their ongoing needs and risks. We promote active engagement in the community and the opportunity to develop new lifestyle choices to improve self-esteem, confidence and lead a good and meaningful life.
Our ethos is built upon a shared belief that the person is central to everything we do, and we ensure we are responsive to an individual’s changing needs, reviewing, and revising plans to meet these needs. We value positive risk-taking opportunities within a context of robust risk assessment and management.
Our care home services work collaboratively with all relevant external stakeholders including community teams, so that individuals receive multidisciplinary care and support.
The team in these services is made up of the Registered Manager, Deputy Manager, Nurses, and Support Workers.
